The Mandatory 2023-2024 Transportation Request Form is to let us know your child needs transportation to home or an alternate location.

In Lyons, we have bus stops for students inside town for grades Kindergarten-12th grade. You can find those locations on this form as well.

This form can be used for the following situations:

1. August, every student riding a bus needs to be accounted for with this form.

2. New students moving into the district at any point.

3. Student needs have changed due to a move or change in sitter
